HDF and Valid ID required to those who will be passing BLISTT on Dec. 16 to 22



The mayors of Baguio, La Trinidad, Itogon, Sablan, Tuba and Tublay (BLISTT), on Monday, December 14, 2020, agreed to require government valid government issued ID and Health Declaration Form (HDF) to everyone passing through borders from December 16 to 22, 2020.

During Entry To and Between BLISTT Areas

“During the BLISTT governing council meeting held on December 7, 2020, it was decided that we implement and observe a temporary enhanced border regulation between the units of the BLISTT, in consideration of the capacity of our hospitals and other health facilities,” stated in the joint advisory signed by the Local Chief Executives of BLISTT.
Also stated in the joint BLISTT advisory that entry to and between BLISTT areas shall be limited to travels for work/official business and for access to essential goods and services. “At border checkpoint, everyone is enjoined to present valid government issued ID as proof of residence with health declaration form,” further stated.

Uniform Health Declaration Form (HDF)

According to the Public Information Office of the City of Baguio, a uniform Health Declaration Form will be uploaded to websites of the LGUs and social media pages, once finalized. The HDF will also be soon available at border checkpoints and barangay offices.

Here are other things you need to know about the uniform HDF:

  • The uniform HDF can be printed, reproduced by users, and filled-up before passing checkpoints
  • Multiple copies needed if passing multiple checkpoints in a day
  • HDF is valid for a day only

Persons to Present Valid ID Only

However, the following are to present valid government ID only:
  • Health workers
  • Uniformed personnel
  • Members of contact tracing teams
  • Members of city, municipal or provincial Covid task force
  • Personnel in LGU and government agencies
  • Cargo and logistics
  • Service providers for water, electricity, telecommunications and ICT
  • Waste management utilities

To Non-Resident Travelers Coming to BLISTT

Non-resident travelers coming in to the BLISTT municipalities from other regions/provinces, on the other hand, shall follow the usual process of online registration at hdf.baguio.gov.ph and upload documents required, according to the Public Information Office of the City of Baguio.
They must have with them their Registration Form (QR Code for Baguio City or Travel Authority for LISTT municipalities) and Medical Certificates issued in their place of origin.
